Is affidavit sufficient for name change in bank and passport?

My wife name has changed after marriage. I have made update to AADHAR CARD. I want to change it in bank accounts, passport. Is an affidavit is sufficient for this or Gazette notification is required?

For getting the name change after marriage, in bank, an affidavit stating the name change along with the marriage certificate is required with other documents like updated Aadhar Card, Pan Card etc., (as per the bank requirements). In Passport, if a is woman applying for change of name/surname in existing passport due to marriage, she must furnish: (i) Photocopy of the Husband's passport, if any, and (ii) An attested copy of marriage certificate issued by Registrar of Marriage OR an affidavit from the husband and wife along with a joint photograph, (Specimen given at Annexure D).
