My parents are separated at my age of 2 year's. My grandfather take care of me and my mother. My mothers maiden name is NANABALA. My father's surname is RAVULA.. At the time of school joining they entered my surname as NANABALA. How can I change my surname NANABALA to RAVULA

  2. Get the affidavit notarized by a notary public.
  3. Publish a notice about the name change in a local newspaper and in the Gazette of India.
  4. Apply for a new Aadhaar card and PAN card with the new surname.
