Property Transfer from mother to children

We are two daughters who have residential property left by our father who passed away 4 years back. The house is in his name. Our mother is unwell and immobile. She would like to transfer her share of property to both our names. Since she cannot step out of home, can the transfer / forfeiting her share be done with power of attorney ?

As the property will be distributed to the legal heirs the mother can transfer the property to legal heirs by preparing the will or by the process of gift  deed.

If the legal heirs are minor then the person can transfer by the way of Power of Attorney.
